DBE Affidavits
Contractors/Consultants must complete and submit DBE affidavit forms to indicate which DBE firms were selected to meet a DBE goal on a contract.
Contractors/Consultants must download and complete the appropriate DBE affidavits for Construction or Engineering Design/Professional Services linked to the Contract Specs and Forms, LPA/Subrecipient Contract Forms and Specs, or Procurement's Federal Forms and Provisions websites.
Note: Engineering Design/Professional Services forms must be submitted with the cost proposal or as otherwise specified in the Request for Qualifications (RFQ).
